Pennsylvania
Assistive Technology Foundation
(PATF):
• Eligibility
– People with disabilities or older people needing Assistive
Technology,
including
home modifications.
• Age qualification – all ages
• Funding Available – $500 to $25,000 if you need
PATF's loan guarantee; otherwise, if you have traditional
credit, you may borrow whatever you need.
• Administered by – PATF Funding Assistance Centers
(FACs) (at webpage below, click on “Funding Assistance
Centers”)
• Vehicle mods – Yes
PATF will also provide
loans to people who don't usually qualify to traditional bank
loans -- applicants must just have a willingness and ability
to repay a loan. PATF provides loans to people of all
ages, income levels and disabilities throughout Pennsylvania.
A partial grant may be available for applicants who want to
borrow up to $1,000 and who have low-incomes (under 14,700/year).
PATF also provides consumers with information about other
possible funding resources.

Housing Assistance Repair Program(SHARP):
• Eligibility
– Section 8 low income. Philadelphia property that is
owner occupied and structurally sound. All utilities functioning.
• Age qualification – 60+ (priority for 75+, living
alone, 130% of poverty, mobile impaired)
• Funding Available – up to $600 for home mod.

Adaptive
Modification Program
(AMP):
• Eligibility
– Philadelphia residents with permanent physical disabilities.
Section 8 income guidelines. Owner- occupant or have owner’s
permission. Property must be structurally sound. Never used
AMP before.
• Age qualification – All ages
• Funding from – from Phila Housing Trust Fund,
Mayor’s Neighborhood Revitalization, CDBG
• Administered by – PHDC (used to be administered
by PCA)

covered by the PA Accessible Housing Program from DCED.

Pennsylvania’s
Initiative on Assistive Technology (PIAT):
This statewide,
federally-funded program is a “one stop” for assistance
in learning about and acquiring assistive technology needed
for home modifications. PIAT's regional resource center staff
can provide information and referral regarding products for
home accessibility; provide or arrange for demonstrations
of home accessibility products; and can provide individualized
intensive assistance in seeking funds from non-governmental
programs.

The PIAT Lending
Library will loan assistive devices to people who would like
to try them, before purchasing, to determine if the device

Rebuilding
Together – Philadelphia (RTP)
RTP’s mission
is to prevent homelessness by partnering with the community
to rehabilitate the homes of elderly, disabled, or low-income
homeowners so that they may continue to live in warmth, safety,
and independence. They provide home modifications for elderly
adults and people with disabilities to make homes safer and
more accessible. Volunteers focus on installing grab bars,
railings, levered doorknobs, and wheelchair ramps. Worn stairs
and floors are repaired. A major partner in RTP’s home
modification endeavor is District V of the Pennsylvania Occupational
Therapy Association (POTA). POTA volunteers join forces with
other RTP volunteers to adapt living spaces so homeowners
can “age in place” and/or remain as mobile and

The National MS Society's Modifications
Assistance Grant (MAG)
The Modifications
Assistance Grant (MAG) is designed to improve accessibility
within the home and community by providing funds toward the
modification of home and/or vehicle. To receive a grant, take
the following steps:
1. Applicant must
have MS and be a registered member of Greater Delaware Valley
Chapter* (For membership information, visit
https://www.nationalmssociety.org/PAE/about/default.asp
2. Complete application
must be submitted
3. Three written estimates must be submitted
4. Assistance from other community resources has been explored.
5. The chapter will not provide 100% of the funding
*The Greater Delaware Valley Chapter
serves the following counties: Pennsylvania: Berks, Bucks,
Carbon, Chester, Delaware, Lehigh, Monroe, Montgomery, Northampton,
Philadelphia, Schuylkill counties and in New Jersey: Atlantic,
Burlington, Camden, Cape May, Cumberland, Gloucester, Salem
counties.
Annual cap: 30% of total not to exceed
$3,000
If additional funding is secured from another community agency
the Greater Delaware Valley Chapter will give an additional
10% raising the assistance to 40% not to exceed $3,000.

Home Modifications
Free Home Modification & Assessment
Programs
JEVS Supports for Independence is
offering three grant programs for low-income seniors living
in the Philadelphia area so that they can remain safe and
secure in their own homes. Simple changes such as installing
motion-sensor lighting, adding shower grab bars and/or the
removal of tripping hazards can help eliminate the risk of
injury and provide a more secure environment.

Free Home Modification and Assessment
programs are available to the following eligible groups:
1. Low-income seniors
who are residents of Chestnut Hill, Dresher, Flourtown, Lafayette
Hill or Flourtown within certain zip codes.
2. Seniors and Consumers
with Disabilities enrolled in one of JEVS-SFI programs for
personal assistance and/or attendant care.
